AI Faces, Unrealistic Expectations, and the Truth About Plastic Surgery

A recent article in Business Insider explored a growing trend that many plastic surgeons are now seeing firsthand: patients arriving with AI-generated images of themselves and asking surgeons to recreate these digitally altered faces in real life.

The article described what many are calling the “AI Face” — flawless skin, exaggerated symmetry, sculpted jawlines, oversized eyes, and proportions that often cannot naturally exist. Artificial intelligence tools can now create a fantasy version of ourselves in seconds. The challenge is that digital perfection and human anatomy are not the same thing.

As a plastic surgeon, I can understand why patients are inspired by these images. People want to look refreshed, healthy, youthful, and confident. Cosmetic procedures can absolutely help achieve those goals. But it is equally important to understand what surgery can and cannot realistically accomplish.

Plastic Surgery Is About Enhancement — Not Artificial Perfection

One of the most important conversations I have with patients today is about realistic expectations.

AI-generated images are often based on impossible standards:

  • Skin without texture or pores
  • Perfect facial symmetry
  • Features that ignore natural anatomy
  • Unrealistic proportions
  • Results unaffected by aging, gravity, or genetics

Human beings are not computer-generated avatars. Nor should they be.

Excellent plastic surgery does not erase individuality. It enhances your natural features while preserving balance, harmony, and authenticity. The best cosmetic results are often the ones that look refreshed and natural — not “done.”

What Plastic Surgery Can Do

Modern cosmetic procedures are more advanced than ever before. Depending on the individual patient, plastic surgery and non-invasive treatments may help:

  • Improve facial definition
  • Tighten loose or aging skin
  • Restore lost volume
  • Improve body contours
  • Reduce signs of aging
  • Enhance confidence and self-image

I like to tell prospective patients that procedures today can often deliver significant improvements with less downtime and more natural-looking results than in the past.

What Plastic Surgery Cannot Do

Plastic surgery cannot:

  • Make someone look like an AI-generated image
  • Completely stop the aging process
  • Create perfection
  • Eliminate every flaw or asymmetry
  • Change someone into a different person

The healthiest and happiest outcomes occur when patients seek improvement, not transformation into an unrealistic digital fantasy.

The Emotional Side of Cosmetic Procedures

Social media filters, edited photos, and now AI-generated imagery are changing how people see themselves. Many patients compare their real appearance to digitally manipulated images that are unattainable in real life.

This creates emotional pressure and distorted expectations — especially among younger patients. My role as a board-certified plastic surgeon ‘role is not simply to perform procedures, but also to educate, guide, and sometimes I say “no” when expectations are unrealistic or emotionally unhealthy.

Ethical plastic surgery always begins with honest conversations.

Beauty Is Still Human

Technology will continue to evolve. AI will continue to influence fashion, media, beauty, and self-image. But there is something important we should not lose in the process: our humanity.

Natural expressions, character, individuality, and even small imperfections are part of what make people beautiful and recognizable.

At the end of the day, my goal as a plastic surgeon is to ensure that my patient does not look artificial or digitally manufactured. I believe that patients should feel more comfortable, confident, and healthy in their own skin.

The truth is that great plastic surgery is not about becoming someone else. It is about becoming the best version of yourself.
